Section 6: Deaths - Causes

Correction 3 August 2016

We regret that the rows were misaligned in the originally published version of Table 6.13. The first three rows of the table were correct but thereafter all rows of numbers should have moved down one row to match up correctly with the labels on the left.  There was also a duplicate row at the bottom of the table.  

We apologise for any problems that may have been caused by this error.
